The patient transfer of care form is required by the EMS Act and Rules and Regulations. The Department requires the primary care provider to provide a report with the receiving facility at the time of the patient's transfer of care. The report must be given verbally and in a written format. This form provides the minimum data elements required to be provided at the time of the patient's transfer of care.
In 2014, Act 139 amended sections of The Controlled Substance, Drug, Device, and Cosmetic Act to permit all certification levels of EMS providers, along with law enforcement officers and fire personnel to administer naloxone during a suspected opioid overdose. The following documents will provide guidance to EMS agencies and providers for operations and patient treatment, as well as guidance for creating partnerships with law enforcement agencies and fire departments.
